Whilst considering the expertise of the Safety & Environment and Occupational Medicine, the International Medicine takes place round the clock for the tertiary Prevention and Treatment that consist to treat or mitigate the effets of a sickness, an accident or an epidemic considering the environment sanitary risks, and the Medical offering in a country where a company is opering. This leads to design a taking charge matrix to optimize the Free Window Treatment ( FWT) that is considered as the maximumum admitting duration before the definitive treatment and then to permit to the patient to be evacuated towards the closest and adapted medical organizaion. This FWT will be strongly dependant of the medical environment of operational sites locations ( sismic, drilling, construction, exploitation ): the qualification of Health Care Specialists of whom the number is correlated to the staff strenght, risks, number of first aiders, equipments and the duration to reach the closest enough medically equipped structures.
A mismanagement of the take charge may have a major psychologic impact on the whole Staff Strenght and subsequently on operations. The problem is different in the middle of the desert, offshore or in a big city as that was recently the case on the site of Mostorod in Cairo.
